We are so excited for you and your kiddo to join us this year in Kindergarten. This is one of our Ages & Stages Questionnaires for Kindergarten Readiness.

The questionnaire can be accessed online from the District 389 school website. If you need help or have questions about any part, please call the school so we can help. We also have a paper version available if needed, so please let us know.
Again, thank you in advance for taking the time to complete this questionnaire. This is part of a state requirement for all students entering kindergarten. Please have this completed by Friday, August 26.
